Patient Packet Instructions:

Welcome Letter – Read the welcome letter and review the fee-schedule.
Medical Release – Please complete and give to your physician who has been seeing you for your condition. Ask them to expedite the record request.
Law Enforcement Letter – Please print and save with your other documents. Law Enforcement is going through a learning phase as well.
Law Enforcement Verification – Please print and save. Any Law Enforcement agent can follow instructions and verify your authorization.
Intake – An intake must be filled out for our record and your appointment. Complete before your visit or you must arrive early to fill out before you see our physicians.
